OptiMed SNF providers EQUIP all facility residents with: Exceptional Quality Under Individualized Plans.

OptiMed offers a presence that supersedes what our competitors offer. We provide medical directorship by an experienced physician that is present in the facility 2-3 days a week for 6-8 hrs a day. There is also an advanced practice provider in each facility 5 days a week. We provide 24/7 coverage by phone when we are not physically present at the facility. In comparison, competitors provide a medical director, on average, one day a week and an advanced practice provider 2-3 days a week.

Current services within the SNFs are, but not limited to, 24-hour skilled nursing care, post-surgical care, wound care management, pain management, IV therapy including IV antibiotics, nutritional monitoring, neurological, cardiac and stroke recovery, physical, speech and occupational therapy.

OptiMed SNF is more than a team, it is a supportive work family. This includes not only OptiMed but our residents too. We have flexible daytime work hours and operate as a mobile rehabilitation, geriactric and long term internist office.
